Please fill in your official institution name and address that should appear in the invoice. Please pay attention to enter correct data. (This is required by Czech tax directives.)
Enter the tax identity of your institution, if any. Czech participants: IČO and DIČ. Foreign participants: VAT number etc.
Enter the description that should appear in the invoice. You can change the pre-filled text according to the requirements of your institution. E.g. "Conference fee, board and lodging for the 57th Summer School on Algebra and Ordered Sets from September 1 to 7, 2019".
Accompanying persons who do not participate at the conference and will be invoiced separately.
Please enter data for the separate invoice for accompanying persons, i.e., customer name, address, tax identity, description.

Consent to the processing of personal data

  1. By filing the registration form the Participant takes note and agrees that his or her personal data provided at the registration form will be processed in accordance with the relevant legislation. The personal data will be processed only in necessary extent for below mentioned purposes:
    • organizing the Summer School,
    • sending invitations to other relevant events organized by Masaryk University (MU), Palacký University Olomouc (UPOL), and the Union of Czech Mathematicians and Physicists (JCMF).
  2. Personal data will be processed only for the time necessary to achieve the above mentioned purposes and the time allowed for archiving.
  3. By filing the registration form the Participant also agrees with
    • the Administrator's right to take photographs from the course and their publication at the Summer School web pages,
    • publishing his full name and affiliation at the Summer School web pages.
  4. The Participant is entitled to claim his or her rights at MU, UPOL and JCMF in accordance with the conditions laid down in the relevant legislation, specifically:
    • the right of the access to the personal data, the right of their correction or erasure, the right of the restriction of processing and the right to claim objection against the unlawful processing;
    • the right to lodge a complaint to the supervisory authority.
